Of course if the pupil is so disruptive, disrespectful, dangerous and/or any number of other despicable behaviours the pupil couldn't careless about their exams, and if they did then they will learn their lesson very early in life " live by the rules" or you are out!
Yes. It's not about one pupil. It's about all the other pupils who have had their work disrupted.
Certainly - a school can exclude any pupil at any time if their behaviour merits it (for a fixed period, or permanently).
