Send an email to SBI bank (clientcomplain) and ask them to issue a new ATM card.
I did it today. Went to the bank, they gave an application for a new ATM Card. Filled it up with my acc. no. and name. That's all. It will be delivered in a month at home by courier. Then go to bank for the password in person with the passbook.
There is no such thing as surrender of old or issue of duplicate card. An entirely new one will be issued to you. You can still use the old one if it is still in working condition.Only on receipt and activation of the new one that you can call up the bank on 1800------no and block your old ATM card. If you do not block it there is no problem except that they will continue to debit the annual charge due for the atm card from your acc. even if you do not use it.

To block your Debit card just sms BLOCK XXXX (Last four digit of your Debit card no) to567676
Though I live far from my home, I am sending them an application with id proof via indian postal service. Lets see what happens. ;)
