I would bring $500 and take back what you don't spend. You can always bring less and use your ATM card to get money. ATM's are everywhere. You should pay in the currency the price is listed to avoid a currency exchange. In other words, if a price is in USD, pay in USD. For example, souvenir shops usually list prices in USD. With that said, it's usually better to pay in colones in most places.
Taxis take colones, although they will take dollars at a big exchange hit. Don't take taxis parked in front of hotels; they won't use a meter and will rip you off. The typical fare anywhere in town should be no more than ¢1500 ($3). Know how to quickly do an exchange in your head so you don't get fooled.
Don't carry more money than you need for the day. Don't bring jewelry or expensive electronics. Wearing jewelry in the street will make you a robbery target. Jewelry and electronics (smartphones, laptops, cameras with shoulder straps). Bring a small pocket camera.
Lunch: around $6 at regular local restaurants, not fancy restaurants

Taxi: it depends, but in general is 570 (around $1.25) colones per every kilometer, for example from Escazu (town near San Jose) to Downtown San Jose it can be around $10 - $15
