U.S. citizens holding a U.S. passport do not need a visa for a tourist stay of up to 90 days. For lawful permanent residents (green card holders) it depends on the nationality whether or not a visa is required prior to travel. The U.S. green card has no relevance in Chile.
Visa requirements based on the nationality can be checked here:
req VISTUR - SI = visa must be applied from a Chilenean Consulate prior to travel
req VISTUR - NO = no visa required for a tourist stay of up to 90 days

I prefer Master Card or American Express when traveling, but Visa can work. JK, likely no as long as it is a short visit.
Not required for stays less than 90 days.
No, but you will have to pay a $140USD entry fee upon entering Chile.
Not if you are staying for three months or less. http://chile.usembassy.gov/tourist_cards...
